The role you’ll contribute:
The Physician Assistant is a professional nurse licensed in Kansas to function in an expanded role to provide primary health care to individuals, families or groups. Nurse Practitioners function in a collegial relationship with physicians and other health professionals in the delivery of primary health care services. They are able to make independent decisions with physicians in carrying out health regimens for families and clients.
The value you’ll bring to the team:
· Work closely with clinical teams and patients and their families to monitor patient outcomes and integrate clinical quality data with payer requirements. Collaborate with physicians, office staff, and management to report data and perform analytics to reflect quality metrics.
· Perform Transitional Care Management services for identified patients.
- Monitor Medicare patients for their Annual Wellness Visits and establish a documented process to assure that patients are contacted about scheduling annual appointments
QualificationsThe expertise and experiences you’ll need to succeed:
- Master’s Degree
- Up to 1 Year of Position-Related Experience
- Kansas Physician Assistant License
- BCLS obtained through the American Heart Association
