The role you’ll contribute:

Responsible for providing a full range of personalized care in the home hospice setting, which may include observation and assessment, direct care and management and evaluation of the care plan to terminally ill patients. Responsible for disease, pain and medication management, as well as, wound care, infusion and various other nursing tasks. Assumes responsibility and accountability for facilitating, communicating and collaborating with both the healthcare team and the patient/family to identify and meet the physical, emotional and spiritual needs of the patient. Facilitates patient / family identified goals for care. Promotes quality end of life care through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrates personal accountability for relationship-based care, organizational mission and core values.

What you bring to the team:

  • Assesses patient’s physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data. Evaluates patient’s response and intervenes to ensure optimal patient outcome.
  • Develops and implements a treatment plan for each patient in collaboration with the physician, patient and the healthcare team. Performs initial evaluation visits as applicable and re-evaluates each patient’s nursing needs on a regular basis.
  • Maintains the comfort and quality of life for those diagnosed with terminal disease.
  • Provides care in a compassionate and understanding manner consistent with hospice philosophy.
  • Conducts hospice visits to assess the patient’s condition. Provides direct patient care, interventions and education as ordered.
  • Demonstrates competency in a variety of therapeutic interventions including but not limited to symptom management, wound care, medication administration and catheterization.
  • Promotes, advocates and collaborates to protect the health, safety and rights of each patient.
  • Provides patient/family education through assessment of patient/family learning readiness, needs and ability. Provides teaching and evaluate effectiveness of teaching.
  • Recognizes the patient or designee as a full partner in providing compassionate and coordinated care based on respect for the patient’s preferences, values and needs.
  • Provides person-centered care in which the nurse strives to understand what is most important to the individual and actively engage them in all aspects of care. This care will be delivered with respect for cultural differences and the diversity of human experience.
  • Delegates to non-RN personnel in accordance with State Board of Nursing and agency policy. 
  • Performs supervisory responsibilities in accordance with regulatory rules and agency policy as required when working with a LPN and/or Certified Nursing Assistant.
  • Participates in quality improvement and safety activities by using prescribed process improvement methods, available evidence, experience, situational judgment and precedents. 
Qualifications

The expertise and experiences you’ll need to succeed:

Minimum qualifications:

  • Associate degree
  •  6+ months of recent nursing experience
  • Knowledge in nursing, evidence-based practice methods and person-centered care concepts 
  • RN License 
  • BLS (American Heart Association) 

Valid Drivers License 

Preferred qualifications: 

  • Bachelor’s degree
  • Previous hospice, home care or oncology experience
